- Antibiotika - 

- Bakterizide:

b-Laktame, Fluorchinolone, Aminoglykoside, Glykopeptide, Ketolide
- Bakteriostatika:
Makrolide, Tetrazykline, Linkosamide, Folsäureantagonisten, Oxazolidinone
- Angriff an der Zellwandsynthese:
- b-Laktam-Antibiotika: Penicilline, Cephalosporine, Carbapeneme, Monobactame
- Glykopeptide
b-Laktam-Antibiotika:

Penicilline:

1. Penicillinase-sensitiv:
- schmales Spektrum: [Meningo-, Strepto-, Pneumokokken, Treponema pallidum; nicht gegen Staph.]
KI: Intrazell.-Bakterien, zellwandlose B. und β-Laktamase
UAW: Allergie, Herxheimer Reaktion, Krampfanfälle
Benzylpenicilline: Penicillin G, (auch als Depot-Penicilline; i.m.)
Oral-Penicilline: Penicillin V
- Penicillin V ist säurestabil (orale Gabe), hat aber schwächere Wirkung auf Bakterien
- erweitertes Spektrum:
Amino-Penicilline: Ampicillin, Amoxicillin (säurestabil, nicht b-Laktamase-fest, RE)
[Gono-, Strepto-, Pneumo-, Meningokokken, Clostridien außer C. difficile]
UAW: pseudomembranöse Kolitis, makulöses Exanthem
Proteus- und Pseudomonas-wirksame P.: Piperacillin (nicht sre.stabil, nicht b-Laktamase-fest, RE)
[Proteus und Pseudomonas, Enterokokken]

2. Penicillinase-fest: Flucloxacillin, Oxacillin (säurefest) [Staphylokokken, MRSA in Kombi]
Kombination mit b-Laktamase-Inhibitoren: Clavulansäure (+ Amoxicillin); Sulbactam
Cephalosporine: b-Laktamase-stabil (Gr. 1 nur gering): Enterokokken sind resistent!

UAW: Allergie, GI ↓, BB-Veränderung, nephrotoxisch

Gruppe 1: Cefazolin i.v. (Basis-C.) [gram+-Kokken; nicht Pseudomonas]
Gruppe 2: Cefuroxim i.v., oral (Übergangs-C.) [gram-: Enterobakterien, Staphylokokken]
Gruppe 3a: Cefotaxim, Ceftriaxon i.v. (Breitspektrum-C.) [schwach gegen Staphylokokken]
Gruppe 3b: Ceftazidim (schwere Infekte, CF) [auch Pseudomonas, nicht Staph.]
Gruppe 4: Cefsulodin (Pseudomonas-C.) [Pseudomonas]
Gruppe 5: Cefoxitin (Anaerobier-C.) [Anaerobier]

Oralcephalosporine:

- 1. Generation (schwache b-Laktamase-Stabilität): Cefalexin [leichte AW- und HW-Infekte]
- 2. Generation (hohe b-Laktamase-Stabilität): Cefuroxim [mitt.-schwere AW- und HW-Infekte, Gonorrhoe]
- 3. Generation (hohe b-Laktamase-Stabilität): Cefixim [schwere AW- und HW-Infekte, Gonorrhoe, HNO]

Carbapaneme (2 Gruppen):

Gruppe 1: Imipenem / Cilastatin [sehr breites Wirkspektrum: alle Bakterien außer MRSA]
UAW: nephrotoxisch, Allergie, Krampfanfall
Monobactame [nur gram--Erreger]

Glykopeptide:
Vancomicin, Teicoplanin i.v. oder oral (bei Penicillinallergie) werden nicht resorbiert, RE
verhindern Bereitstellung von Zellwandbausteinen
UAW: oto- und nephrotoxisch, Red-Man-Syndrom TDM!
[nur gram+-Bakterien: schwere Infektionen wie MRSA, Endokarditis]

- Antibiotika mit Angriff an der Proteinsynthese:

1. Oxazolidinone: Linezolid i.v. und oral (Reserveantibiotikum); keine Kreuzresistenzen; HWZ 7h; HE u. RE
bakterizid [Streptokokken] und bakteriostatisch [Staphylo- und Enterokokken] [auch MRSA]
UAW: RR ↑, Schwindel, KS, Thrombos ↓, GI ↓

2. Aminoglykoside: Gentamycin, Amikacin, Streptomycin; i.v. oder lokal; HWZ 2h, unmetabolisierte RE, TDM
immer kombiniert mit b-Laktam-Antibiotikum!
[schwere Infekte mit gram--Bakterien, Pseudomonaden, Enterobakterien]
UAW: ototoxisch, nephrotoxisch
KI: SS, Kleinkinder, Kombination mit nierentoxischen Substanzen

3. Tetrazycline: Doxycyclin oral oder i.v.; HWZ 15 - 24h;
bakteriostatisch [Mykoplasmen, Borrelien, Chlamydien, H. pylori]
liquor- und plazentagängig
UAW: Wachstum ↓, Zähneverfärbungen, GI ↓, Einlagerung in Knochen, Phototoxizität
KI: SS, Stillen, Kinder < 8

4. Makrolide: Erythromycin (E), Clarithromycin (C), Azithromycin (A) (bei Penicillinallergie)
HWZ: E: 2h, C: 5 - 11h, A: 40h; hemmen CYP3A4 (E u. C) Interaktionen
bakteriostatisch [hauptsächlich gram+-Bakterien: Streptokokken (C: H. pylori!)]

5. Lincosamide: Clindamycin
bakterizid und bakteriostatisch (dringt gut in Knochen ein); oral oder i.v.; HWZ 3h
[gram+-Bakterien: Staph. bei Penicillin-Allergie, Anaerobier, zereb. Toxoplasmose in Kombi]
Kreuzresistenz zu Makroliden
UAW: pseudomembr. Kolitis, RR ↓ bei schneller Spritze, Atemstörung bei Neugeborenen
- Antibiotika mit Angriff an Nukleinsäuren:
- Folsäureantagonisten (bakteriostatisch):
Sulfonamide: Sulfamethoxazol [UAW: nephrotoxische Abbauprodukte]; keine Monotherapie
Diaminopyrimidine: Trimethoprim [UAW: teratogen]; Monotherapie nur bei kompliz. HWI
beides gemischt (5:1) = Cotrimoxazol i.v. oder oral; HWZ 10h, hämodialysierbar
[HW-Infekte, Pneumocystis-Pneumonie]

Resistenzen: Meningokokken, Shigellen, Salmonellen, E. coli
UAW: KM-Depression, Allergien [Lyell-, Stevens-Johnson-Syndrom]

- Fluorchinolone UAW: Arrhythmie (Moxifloxacin), Knorpelschäden, Übelkeit, Erbrechen, KS
- verstärkte Wirkung in Kombi mit β-Laktamen (Cephalosporin bei Pseudomonas)
- Gyrasehemmer bakt. Topoisomerase II, bakterizid; 4 Gruppen:

1. reine HW-Therapeutika
2. Standardchinolone: Ciprofloxacin, Levofloxacin [HWI, AW-Infekte, P. aeruginosa]
3. Fluorchinolone mit erhöhter Wirksamkeit
4. anaerobier-wirksame Fluorchinolone: Moxifloxacin [Kokken, zellwandlose B., Chlamydien]
- Nitroimidazole: hohe BV; HE, HWZ 6 - 10h; Therapiedauer max. 10 Tage
Metronidazol (Bildung reaktiver Metaboliten wo wenig O2 bakterizid)
[H. pylori, Protozoen, Anaerobier]

UAW: teratogen, C2-Unverträglichkeit, Exantheme, KS, Schwindel, GI ↓
beim Tier: kanzerogen

- Antituberkulotika: bakterizid
- Isoniazid (INH): hemmt es die bakterielle Nuklein- und Mykolsäuresynthese
UAW: hepatotoxisch bei langs. Acetyl; neurotoxisch, Optikusneuritis
- Rifampizin: hemmt die bakt. DNA-abhängige RNA-Polymerase [Resistenzen!] auch gegen Lepra
UAW: Leberschäden, Körperflüssigkeiten orange-rot [ Interakt. mit Isoniazid]
- Pyrazinamid: oral; RE; schnelle Resistenzentwicklung
UAW: Leberschäden, Photosensibilisierung, Hyperurikämie
- Streptomycin: Translation ↓
UAW: oto- und nephrotoxisch
- Ethambutol: Biosynthese von Arabinogalactane, Arabinomannane und Peptidoglycane ↓
bakterizid u. bakteriostatisch; langsame Resistenzentwicklung
UAW: Optikusneuritis, Parästhesien, Neuropathie
KI: SS, Stillen, N. opticus-Vorschädigung, Kinder < 10 Jahren
